Located in Wareham, MA, The Tremont Rehabilitation and Skilled Care Center offers skilled nursing and rehabilitative care to its patients and residents. Our staff is devoted to providing our residents with quality care with a personal touch, designed to improve their quality of life. As part of Athena Health Care Systems’ continuum of care, residents can “age in place” knowing that they don’t have to move as their care needs change. Our staff are highly trained to give residents care at every level.
Whether you’re looking for a short stay or to make The Tremont your home, our fully furnished private and semi-private rooms create an environment that is calming and comfortable. Our physicians, nurses and clinical staff, rehabilitation therapists, and ancillary team members will help you every step of the way and will inform you and your family of the progress you are making.
Our skilled nursing services cater to those whose needs require regular 24-hour care and monitoring. This could be short-term for those who have been discharged from the hospital and need extra support time to recover, or those who need traditional longer-term nursing home care.
For our short-term patients, rehabilitative therapy services are available on-site, as often as needed and as prescribed by your physician for optimal recovery and health. Your pathway will include therapy goals, exercises, and specific tasks that will help guide you from your hospital bed to living your best life. Oftentimes, these services include physical therapy, occupational therapy, and speech therapy.

We would like to give Heartfelt Thanks to all the Essential Workers at the Tremont Rehab for their relentless efforts to combat the COVID-19. I have a greater admiration for every person working in a healthcare setting during the corona pandemic because they are not allowed to put their guards down to take a break from the battle. Your bravery and commitment to serve and protect our loved ones is greatly appreciated! A special thanks to Amy Lawrence from Admissions for professionalism and the endless acts of kindness, caring, respect and support she shown to me and my Dad, Edward Jones. She has been beyond essential in keeping me informed and updated on my dad’s health and well being since the lock down began. I am so grateful for that because I know it is not as easy for the nurses or aids to answer my questions or and concerns as they have done in the past. Amy has gone above in beyond the call of duty in my eyes to ensure that my dad would have all that he needed since I couldn’t be there. She has been a lifeline for me to stay connected to my father. These acts of kindness have allowed me to sleep better knowing that my dad was as safe and comfortable as possible.
I want to thank all of you… Kim, Laurie, Emily and all that either I or my husband Leo had contact with during his stay at Tremont. The care and consideration you gave to Leo, and the love with all the hugs you showered on me will never be forgotten. Whenever I came to visit with him, so many of you showered me with hugs and caring. It is something I will never forget. Again, I want to thank you from the bottom of my heart. You did me more good thank you’ll ever know.
The Mohidin family would like to thank all of you who gave our mother, Alex Mohidin, love and support and care during the final years of her life. We greatly appreciate the love and affection and gentleness shown and given to her. It made the difficult decision of placing her in a nursing home easier to bear. We knew deep down in our hearts that the care she was getting was better than we could provide her at home.
Dear Tremont Rehab staff & residents, I wanted to thank you all for how great you were to Patience Frazier over the years. Mike in maintenance, Sandy & Donna in recreation activities, Kayla at reception, Maria & Ariel in the business office, the kitchen & laundry staff (always welcoming me for meals, and delicious meals). I don’t know everyone’s names, but I do know you were loving and kind and we really appreciate all you did to keep her happy.
Here’s a little thank you and appreciation from Sylvania’s children. Since we had to put her here, it has been trouble some to all of us and the family. But, because of you (and others) we are so thankful and grateful for you!
